Atypical and Malignant Meningiomas: Increasing Incidence?
نویسندگان
چکیده
Introduction: Traditionally, the vast majority of meningiomas have been benign lesions with a low propensity for regrowth or spread following gross total removal. Over the past five years, our center has encountered a sharp increase in the percentage of meningiomas with pathological evidence of atypical or malignant features. This report addresses our growing experience with this entity. Methods: We retrospectively reviewed records of all patients evaluated for a meningioma at our center from 2002-2009. Patients followed with serial imaging studies or treated presumptively with radiosurgery without pathological diagnosis were excluded from review. Results: Over an eight year period, we encountered 382 patients with a presumptive diagnosis of meningioma and operated on 187. Of these, 18 (9.6%) had atypical or malignant features on pathological evaluation. In this group, five had previously undergone cranial irradiation (three for childhood leukemia); the remainder had no obvious predisposing factors. Six involved the skull base; the remainder were convexity or parasagittal. Despite aggressive resection, all lesions recurred and required additional surgery and adjuvant radiotherapy and/or chemotherapy. Conclusions: Based on our experience, it is possible that the incidence of atypical/malignant meningiomas is increasing. This may be due to a growing population of childhood cancer survivors, though most patients had no obvious predisposing factors. The referral nature of our practice may have contributed to our encountering more complex lesions; but other than local edema, these lesions did not demonstrate unusual features suggesting malignancy on preoperative imaging. In our experience, meningiomas no longer represents a uniformly benign disorder. This information may be important when counseling patients regarding prognosis, particularly before a pathological diagnosis has been made. Categories: Radiation Oncology, Neurosurgery, Oncology

Surgical Outcome of Treating Grades II and III Meningiomas: A Report of 32 Cases
Aim. To evaluate the frequency of atypical and malignant meningiomas and analyze recurrence rate; to study the morbidity and mortality of these tumors compared to benign meningiomas. Methods. During 1992-2007, 16 patients with malignant and 16 patients with atypical meningioma were operated in Neurosurgery Department of Thessaloniki's Papanikolaou Hospital. We analyzed tumor histology, location...
متن کاملCorrelation of histopathologic classification with proliferative activity and DNA ploidy in 120 intracranial meningiomas, with special reference to atypical meningioma.
Histologic classification of 120 meningiomas was correlated with their proliferative fraction and DNA ploidy using immunohistochemistry and flow cytometry to differentiate histologically atypical meningiomas from benign ones. Histologically, the 120 meningiomas included 101 benign (43 meningotheliomatous, 40 transitional, 11 fibroblastic, 2 secretory, 2 microcystic, 2 angiomatous, and 1 psammom...
متن کاملDNA Methylation in the Malignant Transformation of Meningiomas
Meningiomas are central nervous system tumors that originate from the meningeal coverings of the brain and spinal cord. Most meningiomas are pathologically benign or atypical, but 3-5% display malignant features. Despite previous studies on benign and atypical meningiomas, the key molecular pathways involved in malignant transformation remain to be determined, as does the extent of epigenetic a...
متن کاملDiffusion-weighted MR imaging: diagnosing atypical or malignant meningiomas and detecting tumor dedifferentiation.
BACKGROUND AND PURPOSE Atypical and malignant meningiomas are uncommon tumors with aggressive behavior and higher mortality, morbidity, and recurrence compared with benign tumors. We investigated the utility of diffusion-weighted (DW) MR imaging to differentiate atypical/malignant from benign meningiomas and to detect histologic dedifferentiation to higher tumor grade. MATERIALS AND METHODS W...
متن کاملAnaplastic Meningiomas: a Review
Received, May 3, 2004. Accepted, April 27, 2005. ATYPICAL AND ANAPLASTIC meningiomas are uncommon tumors with a poorer prognosis than benign meningiomas. We reviewed the current literature and attempted to integrate and summarize available information to determine a logical approach to these tumors. Both tumors are rare and are often integrated with benign meningiomas when treatments are evalua...
